The Victoria Youth Clinic Society, lead agency for Foundry Victoria is seeking a registered Master Level Social Work Clinician. Foundry Victoria is an integrated youth and young adult service delivery model comprising primary health, mental health, substance use, social services and peer support. This position works independently and part of a robust interdisciplinary primary care, mental health and substance use team to provide advanced level clinician interventions. This position acts a clinical resource in the area of mental health, substance use and social determinants of health. Participates in intake assessment, case management, care planning, intervention, and consultation with youth aged 12 – 24. Establishes supportive and trusting relationships that promote mental health and wellness and reduce client risk within a stepped care model and in a drop-in basis.
The Social Worker maintains related records and statistics and provides reports to the Executive Director as needed. Reports to the Clinical Coordinator and Operations Manager.
